Why are forms required?
Forms help your provider:
• understand your symptoms and history
• prepare for your appointment
• conduct a more efficient and accurate evaluation
What happens if I don’t complete them?
In most cases:
• your provider can still proceed with your appointment
• any missing information may be collected during the visit
👉 However, completing forms ahead of time is strongly recommended to avoid delays.
Where do I complete my forms?
You can complete forms in your client portal:
• log in to your account
• navigate to your forms or intake section
• complete any required questionnaires
Important notes
• forms are part of your medical record
• completing them in advance improves your experience
• some features (like prescribing) may require completed information
