How do I request my medical records?

You can request access to your medical records at any time.


What records are available?

Your medical records may include:
    •    chart notes from your appointments
    •    diagnosis letters (when applicable)
    •    treatment plans or provider documentation
    •    medication-related information (when documented)

These records are securely stored in your account.


Option 1: Check your client portal first

Some documents may already be available in your portal.

You can:
    •    log in to your account
    •    navigate to your documents or records section
    •    view or download available files

👉 This is the fastest way to access documents that have already been uploaded.


Option 2: Request records from support

If you don’t see the records you need, you can request them from our support team.

Please include:
    •    your full name
    •    the type of records you’re requesting
    •    the date(s) of service (if known)

Our team will review your request and provide next steps.


Can you send my records to someone else?

For privacy and security reasons:
    •    we do not send records directly to third parties
    •    records are shared with you, and you can forward them as needed

How long does it take?

Timing may vary depending on your request, but our team works to fulfill requests as quickly as possible.
